Instructions

1. REQUIRED INFORMATION -- All fields are mandatory. Please fill in all blanks.
2. EMAIL ADDRESS POLICY -- You must provide a valid email address at which you can be contacted. If you check the dot to make your email address private, it will not be displayed when your movie list is viewed (and only the database administrator will have access to it). Email address is requested so that other viewers can contact you if they desire. Email address is required so that I can contact you for details in the event of an error, or to inform you should this database relocate. Email addresses will NOT be sold, traded, auctioned, leased, coerced/tortured out of me, or otherwise given out to another party -- I hate junk email just as much (if not more) than the next guy.
3. NAME POLICY -- You are not required to use your real name, but keep it clean, ok?
4. COST OF SERVICE -- There is no fee for this service, so naturally there is also no warranty. If you experience trouble with this form or any aspect of this database, please email webmaster@dedsune.net, and describe the problem in as much detail as possible (ie. what error message(s) did you receive?).
